Job Description

We have an MS Excel document we use for Sales; it is sent to clients who complete it (usually in PDF) and then print it and fax it back for ordering purposes;

It contains:
- Personal Information like name, address, phone number
- Credit information like Drivers License, SIN, Credit Card information

I would like to expand it to include ie: Shipping information etc and then make it more dynamic (maybe HTML and work through the users browser?!); ie: select 2 credit options, form validation, etc.

When the client opens it, they can complete the form and then print it off for faxing; due to personal credit information, we do not want to allow for email.

Needs to be a single document and not too heavy; am open to suggestions.
